What is a medical book and who needs it

As noted by the portal “Explain.rf”, a personal medical book (LMK) is a document of strict reporting. The form is not sold freely, but is issued only by centers of hygiene and epidemiology and organizations authorized by Rospotrebnadzor. It confirms that the person has undergone a medical examination and has access to a certain job.

The medical book protects three sides at once. Employee – regular medical examinations help to notice the disease in time (for example, tuberculosis in a kindergarten teacher). Consumers – reduced risk of infection through food or personal contact (for example, if the cook is found salmonellosis). Employer – hiring an employee without LMK threatens with a fine and reputational losses.

The book is required by the so-called decreed contingent. This is everyone who works with food, children, water, treats people or provides household services: cooks, sellers, educators, hairdressers, doctors, workers of pools and water supply facilities.

Electronic or paper: what has changed in 2026

Electronic medical books are introduced in stages. The Ministry of Health of the Russian Federation extended the transition period until September 1, 2027. Until this date, paper books of the former sample are issued in parallel, and after that, paper books can be used only until the free sheets run out.

Data on medical examinations are recorded in the unified state system of FGIS “Medical examinations”. Authentication now takes minutes: a paper book is checked by registry (by an eight-digit number), and an electronic book is checked by portal or QR code. The fake will be detected immediately. At the same time, new mandatory fields have been added to ELMK – the gender and citizenship of the owner.

How to make a medical book

If you draw up an electronic book, submit an application through “Public Services” in the “Health” section. Next, receive a referral from the employer (if necessary), undergo a medical examination in the clinic with access to the EGISZ, and then - hygiene training and certification (for example, remotely on the Gigtest portal). When all data enters the system, the book will be given the status of "issued".

For a paper version, choose a clinic with a contract with the Center for Hygiene and Epidemiology.

Standard package of documents: passport, referral from the employer, photo 3×4 (for paper), SNILS and MHI policy.

Liability for absence and forgery

Working without a medical record is an administrative offense. The fine for officials and individual entrepreneurs - from 15 to 25 thousand rubles, for legal entities - from 110 to 130 thousand rubles. The employee is suspended from work without pay.

It’s another thing to buy a fake. For the use of a knowingly forged document faces criminal liability under Part 3 of Art. 327 of the Criminal Code of the Russian Federation: a fine of up to 80 thousand rubles or restriction of freedom up to a year. For the manufacture and sale of fakes - up to two years in prison.

Since all the data is now flocking to the registers, it is impossible to pay for the “crust” and go unnoticed.
