Some women are sure that if the menstrual cycle is regular, then everything is fine with health. But the reproductive system is more complex, and some causes of infertility can remain invisible for years. Russian gynecologist, reproductive therapist Aza Aslanovna explained what to pay attention to when planning pregnancy, writes sb.by.

The doctor noted that a regular menstrual cycle can cover all issues, if we talk about the well-being of a woman:
- But the same processes that are responsible for the hormonal background, the condition of bones, hair and cognitive functions, in parallel, are the basis for motherhood. And it's important to distinguish. Within the menstrual cycle, there are also ovarian and uterine cycles. In order for menstruation to occur, a follicle matures in the ovary, which provides a hormonal profile. If the process ends with ovulation, there is a chance of pregnancy. But it happens that the follicle matures, grows the endometrium, creates a hormonal background, and the rupture of the follicle does not occur. The woman will not notice this without additional examination.
According to the reproductive therapist, the patency of the fallopian tubes is one of the key factors that cannot be assessed by sensations:
- When the egg leaves the follicle, it must enter the fallopian tube, which is able to attract it. And then the sperm must be accessed into this tube so that the sperm and egg meet. The patency of the fallopian tubes is something that we evaluate, and something that can not be realized at the level of a woman’s feelings. Pipes can be impassable, it does not hurt and does not bother, only disrupts reproductive function. You'll feel great.
She also stressed the importance of the male factor:
- Unfortunately, very often the fertility of the spouse is pushed to the last plan. Historically, we consider men to be perfectly healthy automatically. This is not the case, and half of the problems associated with fertility in a couple are taken away by the male factor. A man and a woman in case of problems with the onset of pregnancy should together come to the doctor for examination.
The gynecologist explained how the condition of the uterus is assessed:
- At the macro level, we also assess the state of the uterus. There may be fibroids, endometriosis, polyps. The question is solved simply: by the beginning of the menstrual cycle, you come to the appointment, and the doctor assesses that the uterine cavity is normal or there are deviations that need to be eliminated.
Separately, the doctor dwelled on the causes of ovulation disorders:
- The main and most common cause is polycystic ovary syndrome, which has recently been renamed polyendocrine metabolic emergency syndrome. A woman in the ovaries does not have cysts, but there are a very large number of small follicles, among which there is no choice of dominant. No one grows or reaches the stage where a mature egg can come out. The second point is extreme exhaustion of the body, when hormonal levels are disturbed due to low body weight. This is diagnosed at the reception and on a blood test. There is also exhaustion of the ovaries, but there you will feel bells: a violation of the cycle, dry skin, urethral symptoms, dryness during sexual intercourse, difficulties with concentration.
According to her, there are risk groups for obstruction of the fallopian tubes:
- In the risk group, women with endometriosis who underwent abdominal surgery when there was entry into the abdominal cavity and blood could lead to adhesions. Another point is sexually transmitted infections, especially chlamydial, which affects the fallopian tubes. These are the three main reasons I'm trying to figure out in the initial exam.
The reproductive therapist did not ignore the topic of age:
- As a reproductive therapist, I don’t like to talk about age, because we are absolutely powerless. Evolutionarily, women entered motherhood early, and there were almost no problems with the first child after the age of 35. Now women are socially adapted, building careers and delaying motherhood. But observations of natural fertility show that after 35 years, the quality of eggs suffers sharply. More and more of them are not able to give rise to a healthy embryo and viable pregnancy.
She also explained why you can’t rely on the old results of surveys of men:
- It is believed that if a man has a 20-year-old child, then everything is fine with him. It doesn't work that way. Male fertility can be disrupted at any time. If there is no confirmation that a man’s performance is good throughout the year, you can not rely on old results. It is important to evaluate in dynamics. If infertility has begun and you have been dealing with the problem for 10 years, a man should retake the indicators once a year. They can get better or worse. And the spermogram itself is not a diagnosis - you need a consultation of a narrow specialist based on fresh tests.
